        DJS FORMS AVAILABLE ON THE AOPC HOME PAGE

Several DJS forms can be downloaded and printed from the
AOPC Home Page on the Internet. All forms that can be
accessed at the AOPC Home Page are approved by
the Court Administrator of Pennsylvania.  The forms are
available in both PDF and MS Word formats.  

Two of these forms, the Search Warrant and the Police
Criminal Complaint, can only be accessed by police agencies
with a valid ORI number.  

The other forms, the Civil Complaint, the Landlord and
Tenant Complaint, and the Private Criminal Complaint can be
downloaded and printed by any person who accesses the AOPC
Home Page.  With regard to the Private Criminal Complaint
form, please be advised that Rule 106 of the Pennsylvania
Rules of Criminal Procedure requires that when a private
citizen completes this form, it must be submitted to the
District Attorney's Office for approval before it can be
filed in a Magisterial District Court.  If a citizen has any
questions in that regard, he/she should contact the District
Attorney's office.
