FAXING DOCUMENTATION Remember that you need to include your court number, your name, and the problem number with faxed documentation. Address the cover sheet to the Help Desk. If you know it, please include the Help Desk operator's name. REQUESTS FOR EXPUNGEMENT AND CHANGE OF USER INFORMATION Expungement requests need to be filed on the Request for Expungement form (AOPC 101). This form is available on the Print Blank Forms option on the Printer Functions Menu. Complete the form and attach a copy of the court order. You do not need to attach a separate cover letter. Requests for adding a new user, deleting a user, or changing user information must be submitted on the User ID and Password Request form (AOPC 100A). This form is available on the Print Blank Forms option on the Printer Functions Menu. The completed form must include an original District Justice signature and seal and be mailed to the AOPC. Faxed forms are not accepted. PREPARING TO CALL THE HELP DESK 1. Remember the steps you have taken. 2. Have a pen and paper available. 3. Have the case information with you. 4. Have the case selected on the screen. 5. Know the page number of the manual you have consulted.
