               SURETY INFORMATION PAGE (AOPC 414A5)

The signature line on the Surety Information Page (AOPC 414A5) 
has been changed from "District Justice Signature" to "Clerk 
of Courts or Issuing Authority."  This conforms with the 
signature line on the Bail Bond (AOPC 414A1).
