Instructions for Downloading the Police Criminal Complaint Forms

In order for forms to be downloaded successfully, the following steps must be completed properly on the log-in screen:

• The ORI must be entered in the ORI Number field with the letters PA followed by seven digits. There must be no spaces between the letters and the digits. Example: PA0000573

• The phone number entered in the Phone Number field must be the one that PSP has associated with the ORI. There is only one valid phone number per ORI.

• Phone numbers must be entered with NO dashes, parentheses or spaces between the numbers.

• Type your department's e-mail address in the E-mail Address field. If you have no available e-mail address, type N/A in this field.

Please note that these forms are password protected and cannot be altered.
