Provide attorney's full name, date of admission, name of person who moved admission, and the address where the wall certificate should be mailed. Effective February 1, 2000, the fee is $50.00, payable in advance. Checks should be made payable to the Supreme Court of Pennsylvania.

Turnaround time is approximately four to six weeks, depending on demand.
